In the first 15–30 minutes, establish exactly which document is missing, save any copy you have, and separate three tasks: reporting the loss, protecting your data and obtaining a replacement. For a residence card, reporting the loss and applying for replacement are separate actions with different deadlines. For a foreign passport, the main route is through the authorities of your country of citizenship, rather than a Polish passport office.

If your wallet or phone was stolen with your documents, contact your bank immediately through an independently verified channel. Report the circumstances of theft to the police and ask for confirmation. If the passport was simply lost, a police document may be required by your consulate, but there is no single procedure covering every country's passports. Do not delay notifying the Polish card authority while waiting for a consular appointment.

Checked on 18.10.2026. Next scheduled review — 18.11.2026. This guide covers a passport, a residence card, both documents together and a child's documents. A shorter card-only route appears in our separate checklist. Here, consular procedures, establishing identity and the ability to travel also matter.

First steps: loss, theft and travel

Identify your situation before booking an appointment
Situation Do now Next step
Foreign passport lost Check copies, number and circumstances Contact your own consulate about its loss procedure
Karta pobytu lost Identify the issuing authority and complete the notification Apply separately for replacement
Passport and card stolen Police, banking security and notification to the card authority Consular identification and Polish replacement in parallel
Document missing before a flight Do not assume a certificate replaces it Check route-specific documents with the competent authorities and carrier
Child’s document missing Keep evidence of parentage and representation Consulate and card authority for the relevant category

Record the last known date and location, document type, issuing authority and likely circumstances. If you do not know whether it was stolen or lost, say so: do not turn a suspicion into an invented fact. A copy helps locate a record and establish identity, but does not become a valid original.

Contact the lost-property office, carrier or place you last visited when appropriate. Searching does not suspend the statutory reporting deadline. Do not publish your passport identity page, card number or PESEL on social media. An announcement about found property can provide a contact without disclosing someone else's personal details.

Immediate danger, violence or an ongoing crime requires urgent help. For ordinary document replacement, use the relevant authority rather than treating an emergency line as an administrative helpline. Do not pay a stranger promising to “remove the document from the database” or produce a duplicate outside the official process.

Loss and theft: when to contact the police

For theft, describe the event at a Policja station: location, time, missing items and known circumstances. Ask for confirmation that the report was received, and keep the case number if one has been assigned. Depending on the procedure, this may help with the consulate, residence authority, bank or insurer.

For accidental loss, the significance of police confirmation depends on the passport's issuing country. For example, the official Ukrainian instructions require contacting the local police and obtaining a document confirming the report. This cannot be extended to every foreign passport, nor can anyone promise that every police unit will issue an identical certificate with your preferred wording.

The police do not issue a replacement foreign passport or print a residence card. A theft report does not replace notification to the authority that issued the Polish card or an application to replace it. The Polish online service for reporting a lost Polish passport is not a universal service for foreign passports either.

Foreign passport lost: your consulate and identity checks

Find the specific instructions from your embassy or consulate: getting a new passport abroad and obtaining a temporary return document are often different services. Explain where you are, your citizenship, whether you have another valid passport, and whether you intend to return home or continue living in Poland.

Depending on your country's rules, identification may involve a surviving national identity document, a copy of the missing passport, previous issue records, a birth record, national registry data and other evidence. A screenshot alone does not guarantee immediate issue. If you have no documents at all, ask about identity verification rather than accepting a fabricated account for the application.

A full new passport usually involves more steps than a document allowing you to return home. A temporary document may have short validity, a limited purpose and specific transit conditions. It does not automatically provide grounds for another entry into Poland. Before buying a ticket, confirm an acceptable itinerary and the documents needed for transit.

If the consulate is far away, first check its territorial jurisdiction, appointment system and emergency arrangements. Correspondence may help you obtain instructions, but cannot replace mandatory personal identification. Do not send originals to a random intermediary's address. Keep official replies and evidence of contact together with your residence decision.

Ukrainian citizens: a new passport or a return document

According to the Ukrainian foreign ministry's official guidance, a passport lost abroad should be reported to the local police, confirmation obtained and the Ukrainian diplomatic mission contacted. Having a second valid international passport changes the practical route, but the loss of the first must still be reported under the applicable rules.

To continue living abroad, establish how to obtain a new passport from the competent Ukrainian office. Do not confuse a consular procedure with the services of an overseas DP Dokument branch: channels, appointments, checks and supporting documents may differ. Before paying, confirm through your selected official channel that it handles your particular loss situation.

A Ukrainian identity certificate for return is for returning to Ukraine, rather than unrestricted journeys and re-entry into Poland. Where documents are missing, the consulate verifies identity and citizenship. A child's case requires the legal representative's documents and evidence of parentage or authority; a child missing from a digital service must not be represented using another person's details.

Additional registry checks apply to some adult applicants. Ukrainian Resolution No 981 of 29.07.2026 regulates the relevant consular procedure and data updates. Do not rely on outdated blanket advice that “everyone gets a passport without checks” or “everyone must travel to Ukraine”. Check the particular service, current records and available exceptions; a return document is considered separately from ordinary passport issue.

PESEL UKR status is not an international passport. Losing the physical booklet does not in itself mean temporary protection ends, but identity records must remain up to date. Report new passport details to the competent authorities under the applicable procedure. Do not assume the Polish register automatically updates when Ukraine issues a new document.

If a child loses a passport, first establish who may apply, which documents prove representation and whether the child's personal attendance is required for that service. Adult requirements must not simply be imposed on a minor. After issue, check the name, transliteration and date of birth before the next administrative step.

Other nationalities: consular services are not interchangeable

Verified routes and their limits
Citizenship / situation Official route Important limitation
Ukraine Ukrainian foreign ministry: passport services and return certificate Passport replacement and return are different services
Russia Consular section in Warsaw: entry (return) certificate It does not automatically permit another entry into Poland
Belarus with an eligible Polish status UdSC: a special Polish document for a foreign national Status and conditions matter, not just citizenship
Another country Competent mission of your country of citizenship No verified universal deadline or fee for everyone
International protection / protection application Competent Polish authority and legal assistance Do not automatically seek protection from the authorities of the country of persecution

The Russian consular section in Warsaw publishes a separate procedure for a return certificate: personal attendance, an application, photographs and identity evidence from its stated list. An internal passport or the prescribed witness statements have their specific role; a copy of an international passport is not sufficient by itself. Additional verification is needed without supporting evidence. Check appointments with a mission that actually operates, rather than using an address of a closed consulate from an old article.

Belarusian citizens cannot be promised ordinary national passport exchange at a consulate in Poland. A different, verified Polish route matters: the procedure under Article 252a has been extended until 31.12.2026. It covers Belarusians whose document was lost, damaged, expired or never held, with the statuses listed in the law: a temporary or permanent residence permit, EU long-term resident status, subsidiary protection or humanitarian stay. Merely being in Poland is insufficient.

A Polish document for a foreign national is not a Belarusian passport and is not issued automatically to everyone who reports a loss. Visa availability and border crossing also depend on document recognition and your grounds for entry. Extending the procedure until the end of the year does not confirm an extension for the following year.

If you are applying for international protection or already have it, contacting your country of origin's authorities and using its passport may affect your situation. First agree the appropriate route with the competent Polish authority and independent legal assistance. This is a separate branch, not an instruction that everyone must go to their own embassy.

Lost karta pobytu: notification within three days

For an ordinary third-country national's card, Article 232 of the Foreigners Act requires notifying the issuing authority within 3 days of loss or damage. This is usually the voivode, or for certain residence grounds the relevant Straż Graniczna authority. Check the name on the card copy or in the case documents.

Use the prescribed notification form rather than only a free-form letter. The current route and links to forms appear in MOS: Karta pobytu. Enter the real circumstances, card details and contact information. If the number is unavailable, do not invent it: ask the authority to help identify the record.

Keep proof of delivery. Electronic submission requires the official channel offered by the authority and a properly signed document; an ordinary email to an information address should not automatically be considered sufficient. Booking a future appointment is not the notification itself either. Do not wait for a convenient appointment when the deadline is already running.

After notification, a free zaświadczenie confirms that loss or damage was reported. It remains valid until replacement, but no longer than 2 months. It confirms an event and procedure; it is not a new passport, a new card or general permission to travel.

People use “residence card” informally for different documents. A karta pobytu held by a third-country national is not the same as a karta pobytowa held by an EU citizen's family member. There is also a karta stałego pobytu for an EU citizen's family member. Calling a card “permanent” is not a reason to use another category's form.

For an EU family member's document, the separate MOS instructions for karta pobytowa also provide for notifying the issuing voivode within 3 days and applying separately for replacement. That does not mean the ordinary foreign national's card fee applies. For an EU family member's permanent card, refugee documents and other categories, select their own section and competent authority.

First copy the exact document name and basis of issue, then complete the application. If only the decision survives, provide its number and date. Our short explanation of the card helps distinguish the document from the right to stay, but cannot replace instructions specific to your category.

A new card: separate application, documents and cost

An ordinary card is replaced on application, not simply because its loss was reported. Article 230 sets a deadline of 14 days after the replacement ground arises. Notification within 3 days and application within 14 days do not replace each other. They can be handled during the same period, but make sure both actions are completed.

Replacement concerns the document. It is not a new temporary residence permit and does not automatically extend the previous permit. If that is expiring soon, handle further residence separately. Since April 2026, applications for temporary, permanent and long-term resident permits use the new MOS; this must not be confused with replacing the plastic card. The official explanation specifically retains the service arrangements for card cases.

What to prepare for ordinary replacement
Item Purpose What to check
Issue / replacement form Start the card procedure Ground: loss, not a new residence permit
Valid travel document Identification Original and required copy; exceptions assessed individually
Photographs Card production For the described voivodeship procedure — two, 35 × 45 mm, from the last 6 months
Notification / confirmation Link the case to the loss Date, authority and proof of delivery
Payment confirmation Document fee Correct account and ground, not a new permit fee
Additional documents Confirm details and circumstances According to category, address and representation of a child
Personal attendance / biometrics Verification and production Summons and exceptions under current rules

An example of the current checklist, photograph requirements and forms is published by the Łódź Voivodeship Office. Another competent authority may organise submission channels and biometrics differently. Do not order a new package using an old checklist if your category requires something else. Keep a damaged card for return rather than throwing it away.

Ordinary karta pobytu fees: circumstances of loss matter
Ground Amount Qualification
General issue / replacement 100 PLN Unless a higher fee, reduction or exemption applies
First loss / damage attributable to the holder 200 PLN Fault matters; theft is not automatically culpable loss
Subsequent culpable loss / damage 300 PLN Not the universal fee for every later replacement
Notification certificate 0 PLN Maximum 2 months; not an entry document
Photographs, translation, consular services Separate Not automatically included in the Polish card fee

Reductions and exemptions have specific grounds. Do not extend a first-issue exemption to every later loss or promise every child one fixed final amount. Supply evidence of any reduction ground and request an explanation of a disputed higher fee. Official evidence of theft helps describe the circumstances but does not by itself guarantee any particular fee outcome.

Production time for a new card is not the reporting deadline. Checking documents, identification, biometrics and printing take time; there is no verified universal promise of completion “in a few days”. Watch for requests and check progress using the case number. If the certificate is expiring, contact the authority in advance: do not change its date yourself.

Keeping the case on track between application and collection

Create a separate case folder: notification, proof of delivery, replacement application, payment, replies and summonses. Each action should have a date. A submitted but unsigned draft is not a proper filing. An appointment booking number does not prove acceptance of an application. If a representative submitted the documents, obtain a copy of what was actually filed.

Do not confuse the card-fee account with the residence-permit-fee account. Put the details required by the authority in the payment reference, keep confirmation and check the recipient. For an incorrect payment, ask how to correct it or obtain a refund. Paying again into a random account in an unknown “official's” email will not resolve the issue.

Read any request carefully to see what is missing: a passport copy, identity information, photograph, payment evidence or personal attendance. Replying “I already sent everything” without the attachment and evidence may not remedy the deficiency. If you lack the document, explain why, attach proof of consular contact and ask about the acceptable procedure rather than leaving the letter unanswered.

When your actual address changes, arrange to receive correspondence and update contact details in the case. An unread notice should not be treated as nonexistent merely because you no longer live at the old address. Check the official electronic inbox if that is the delivery channel; a readiness text message is not a substitute for reading procedural documents.

At collection, compare the surname, given name, date of birth and other details with the application. Show any error to the officer immediately and keep evidence of reporting it. Do not alter the plastic yourself. A technical error is a separate replacement ground; it must not be confused with another culpable loss and the corresponding higher fee.

For a child, keep evidence of parentage and representation in the folder. If someone other than a parent applies, authority must be established rather than merely attaching a note saying “the family agrees”. Explain different surnames after marriage or different transliterations with documents. That assists identification, but does not mean any collection of documents will be accepted unchecked.

Passport and card missing together

  1. Save copies and issue details. For theft, report it and secure payment instruments.
  2. Notify the card authority promptly using the prescribed form.
  3. Contact the consulate for identification and a choice of passport or return document.
  4. Apply for card replacement within the deadline and explain the missing passport with supporting evidence.
  5. Agree which identity evidence the authority can accept in your circumstances and when to supply the new passport.
  6. After collection, update relevant cases and check details before travelling.

Article 231 allows another identity document instead of a travel document in a particularly justified case where the latter is missing or cannot be obtained. This is not an automatic exemption for everyone who loses a passport and does not permit an invented substitute. Provide an explanation and available evidence; the outcome depends on the circumstances.

A surviving residence decision, case number and copies help but do not guarantee completion without identification. If consular checks take longer, inform the Polish authority in writing about your efforts. Do not ignore a deadline to provide further documents: explain difficulties promptly and ask about the permissible next steps.

Residence and work without the physical document

Losing the plastic card does not in itself cancel a residence permit. But a missing document and an expiring permit are different questions. Keep the decision, card copy, certificate and replacement confirmation. These help explain the situation, but cannot promise the same result in every check as a valid document.

If already employed, check that your residence and work grounds remain valid, alongside permit conditions and the employer's records. Do not conclude that a lost plastic card automatically terminates an employment contract. Equally, an employer should not treat any case number as sufficient proof of a right to work.

For new employment, both rights must be established before work starts under the applicable rules. A “labour-market access” annotation on a copy does not create an unconditional right to any role with any employer. Give the employer accurate information and agree acceptable evidence. See also our legal-work checks.

If the passport changes, establish how to update its details with the employer, in pending proceedings and relevant services. Do not send a full scan to an entire HR department through an open group chat: use the designated secure method. Losing a document should not cause another data leak.

Leaving and returning: a certificate is not a border-document set

Under Article 242, a valid karta pobytu together with a travel document permits repeated border crossings without a visa. A loss certificate alone is not that combination. A residence decision, card copy and phone photograph do not automatically become the plastic card needed for return either.

Travel within Schengen requires the appropriate valid documents. The absence of routine internal border checks does not remove requirements: checks and temporary border controls remain possible. Airlines check documents separately. Carrier approval does not create a right of entry, and a general telephone answer does not replace assessment by the competent authority.

Before flying without the card, assess your own grounds for returning: a valid visa, an applicable visa-free regime with its conditions, or another lawful route. Having retained your passport does not automatically make every journey safe. If the grounds are unconfirmed, postponing departure is wiser than relying on an explanation at the check-in desk.

A return document for your country of citizenship usually solves that particular journey. Check transit countries, whether you must leave the international transit area and ticket requirements. After returning home, you may need a new passport and separate grounds to enter Poland again. Do not buy a complicated itinerary on an intermediary's promise that “this certificate gets you in everywhere”.

Protecting data and dealing with a found document

After wallet theft, tell the bank about the payment card and document, and check transactions and access. PESEL restriction is also available to an adult foreign national with PESEL who meets the service conditions: choose the applicable channel and identity document. Do not treat Poland's online identity-card invalidation service as cancellation of a foreign passport.

A restricted PESEL does not replace residence-card notification, refund money or automatically block every service. Use an independently verified bank contact for suspicious messages. Detailed actions appear in the fraud guide.

If the card is found, Article 233 requires notifying the issuing authority within 3 days. If a new one has already been issued, return the found old one immediately. Do not decide yourself that the previous card can again be used for travel: check its status and possible cancellation. Reporting recovery does not guarantee restoration of validity.

Do not use a found foreign passport until its status has been established with your country's competent authority either. An invalidated passport does not become valid because it has been retrieved from a bag. If theft was reported, also report the discovery within the police case. Hand someone else's document to an authorised body rather than photographing it for a public search for its owner.

Where to go: summary table

Six practical situations

These are hypothetical examples, not actual client stories.

A Ukrainian citizen loses a passport in Gdańsk

First save a copy, contact the police under the Ukrainian instructions and reach the competent Ukrainian mission. Choose a new passport for life abroad or a return document. The difficulties are identification and appointments, not getting a “Polish duplicate”. Basis: Ukrainian foreign ministry.

Card lost, passport retained

Identify the issuing authority, notify it within 3 days and apply separately for replacement within 14 days. Prepare the passport, photographs and appropriate fee. The challenge is not confusing the two actions or fee grounds. Basis: Articles 230–232.

A bag containing both documents is stolen

Police reporting, banking security and card notification do not wait for a new passport. Consular identification proceeds alongside the replacement application; explain the missing passport in writing. The special identification exception is not automatically guaranteed. Basis: Article 231 and consular instructions.

The card disappears before a journey

Complete notification, then check specific grounds for departure, transit and return. Do not treat the certificate as a card. If a safe documentary route cannot be confirmed, postpone the journey. Carrier requirements and border requirements are not identical. Basis: Article 242.

A document is found after reporting it

Do not throw it away or use it immediately. Notify the card authority within 3 days, establish status and return the old card after a new one is issued. For a passport, contact your country's authority; for theft, also the police. Basis: Article 233 and national passport rules.

A parent loses a child's documents

Keep the parent's own document, birth certificate and evidence of authority; notify the relevant bodies. Check the child's attendance and biometrics separately. Adult fees and consular checklists cannot be transferred automatically. Basis: card procedure and the country of citizenship's rules.

Polish messages: supplements to official forms

With the notification: Zawiadamiam o utracie karty pobytu w dniu [data]. Okoliczności: [zgodny z prawdą opis]. W załączeniu przekazuję wymagany formularz. Proszę o wydanie zaświadczenia potwierdzającego zgłoszenie.

To the office: Proszę o wskazanie właściwego trybu wymiany karty pobytu i wymaganych załączników w mojej sytuacji. Posiadam [dokumenty]. Utraciłem/am również paszport i podjąłem/podjęłam działania w celu uzyskania nowego dokumentu.

About progress: Proszę o informację o stanie sprawy nr [numer] dotyczącej wymiany karty pobytu, ewentualnych brakach oraz sposobie odbioru dokumentu. Proszę o wskazanie, czy potrzebne jest moje osobiste stawiennictwo.

These texts do not replace mandatory MOS forms, and simply saving a file does not submit a notification. Use the current form linked for your card, sign it and submit through an official channel. Do not include unnecessary child data or a complete set of sensitive identifiers in public correspondence.

Verified forms from the Bydgoszcz voivodeship portal: loss or damage notification and card issue / replacement application. They are different forms. Check the document category and your authority's current instructions: another voivodeship's form does not change territorial competence. Before sending, check the signature, attachments and recipient address; keep an exact copy of the submitted package.

Frequently asked questions

What is the card-loss reporting deadline?

For an ordinary card, notify the issuing authority within 3 days of loss or damage. The separate replacement application deadline is 14 days. An appointment booking and police report do not replace these actions.

How much does replacement cost?

The general fee is 100 PLN, culpable loss 200 PLN and a subsequent culpable loss 300 PLN. Specific reductions and exemptions exist. Do not confuse card categories or automatically classify theft as culpable loss.

Are the police always required?

For theft, act according to the crime circumstances. For accidental passport loss, requirements depend on the issuing country; Ukrainian instructions require local police confirmation. There is no universal rule for all passports.

Can I continue working?

Losing the plastic does not itself cancel the permit, but residence and work must be evidenced separately. Job and employer conditions remain relevant. Provide accurate information and agree the documents.

Can I fly home?

Check a valid travel document, itinerary and carrier rules. A return certificate may resolve the journey home, but not arbitrary transit or re-entry.

Can I return with only a certificate?

There is no guarantee. A loss certificate does not replace a valid card with a passport. You need a confirmed applicable entry-document set; do not plan on a promise alone if uncertain.

What if I have no passport?

Start consular identification and notify the card authority at the same time. Another identity document may be accepted for replacement in a particularly justified case, but the exception is not automatic for everyone.

Can I use a found card?

Notify the authority first and check validity. An old card found after a new one is issued must be returned. Withdrawing a report does not automatically restore a cancelled document.

Does PESEL UKR replace a passport?

No. It concerns status and registers, but is not a universal travel document. Handle passport loss under the Ukrainian procedure and update Polish records separately.

Can confirmation be obtained online?

The authority may offer an official electronic channel. The correct form, signature and delivery are required; ordinary email should not be assumed sufficient. Keep submission evidence.

Is the certificate valid until the new card is ready?

Until replacement, but at most 2 months. Contact the authority in advance if waiting longer. You cannot extend it yourself or use it as a card at the border.

What happens for a child?

A legal representative acts with evidence of authority; attendance, biometrics and consular documents are checked for the age and service. Do not apply an adult checklist without checking.

Will a Polish office issue a new national passport?

No: ordinary national passport replacement belongs to the country of citizenship. Polish travel documents for foreign nationals have their own eligibility grounds and are not issued to everyone merely because of loss.

Does losing a card extend the permit?

No. Replacing the physical document is not a new permit and does not itself extend residence. If the underlying right is expiring, handle the next status separately from production of the plastic.

Checklist by urgency

Now: identify the document and circumstances, save copies, secure banking and report theft to the police, and suspend risky travel plans until the document set is checked.

Over the next few days: complete mandatory card notification, apply separately for replacement, start the passport route, and retain official confirmations and deadlines for further documents.

Until collection: monitor requests, the residence basis and certificate validity, and agree documents with the employer. After issue, check the details, report new identifiers wherever required and only then return to planning border crossings.

Sources and limits of verification

The core rules were checked against the Foreigners Act, MOS, official voivodeship instructions and consular sources, with links beside the relevant actions. No common consular deadline or fee for every nationality, or universal acceptance of every temporary document by every country, has been verified. No such promises are therefore made. International protection cases, lack of identity evidence and disputed entry require individual assessment. New legislation and authority announcements require an unscheduled update.
