Using NFZ-funded healthcare requires both entitlement to the relevant services and an appointment actually covered by an NFZ contract. A Polish PESEL, residence card or private medical subscription does not by itself establish both conditions. A clinic can offer paid appointments and publicly funded ones at the same time.
The ordinary route is: establish entitlement → choose POZ → see your family doctor → obtain clinically justified tests or a referral → book the appropriate consultation. A life-threatening emergency does not follow these steps in order: emergency assistance is needed.
Checked on 22.10.2026. This explains how to organise care, not individual diagnosis or treatment. Ukrainian citizens should particularly note the changes from March 2026: the old promise that any PESEL UKR gives access to every service no longer works.
What NFZ covers and why not everything is free
Narodowy Fundusz Zdrowia funds services included in the public system at providers with the relevant contract. Entitlement does not mean any procedure you choose, every medicine without a co-payment or any private clinic. Dentistry, rehabilitation, spa treatment and reimbursed medicines have their own lists and conditions.
When booking, explicitly say “w ramach NFZ” and confirm that the chosen doctor has publicly funded appointments in that specialty. A sign saying “we work with NFZ” is insufficient: the contract might cover another service. Before agreeing to a paid test, establish why it is chargeable and whether a covered route exists.
Do not refuse urgent assistance because of uncertainty about payment, but do not assume every SOR attendance is free for every person without an entitlement basis. Emergency treatment and subsequent charging are different matters. For planned treatment, establish entitlement and funding before the service.
The basis of a foreign national’s healthcare entitlement
| Situation | What to check | Common mistake |
|---|---|---|
| Umowa o pracę | Employer registration for compulsory insurance | Assuming signing the contract is enough despite a registration error |
| Umowa zlecenie | Whether this particular contract attracts a health contribution | Automatically treating a student under 26 as insured |
| JDG | Eligible basis, registration and contributions | Confusing a private subscription with NFZ insurance |
| Family member | Eligibility and actual notification by the contribution payer | Expecting automatic inclusion after marriage or birth |
| Foreign student | Own basis, family, university or category-specific NFZ agreement | Applying any Polish student arrangement to every foreign student |
| Registered unemployed person | Whether an applicable insurance basis arose | Treating any unemployment as insurance |
| UKR temporary protection | Current special category or own insurance | Relying on an old blanket special-act promise |
| Insured in another EU/EFTA state | Coordination document and purpose of stay | Treating EKUZ as payment for any planned treatment |
With an employment contract, ask the employer to confirm correct registration. Zlecenie has exceptions: for example, a student under 26 generally does not acquire the same insurance basis as an ordinary employee. Check your status and combination of contracts rather than drawing conclusions from the amount paid into your account.
For a sole trader, public health insurance and a commercial clinic package are different obligations. A family member included in insurance should not have a competing compulsory basis of their own. After acquiring their own insurance and losing it, family inclusion may need a new notification rather than automatically returning.
The ordinary family route uses the contribution payer, such as a parent's employer, and ZUS ZCNA. Do not confuse insurance with birth registration, school enrolment or a PESEL number. Our child-insurance checklist explains the practical steps. In a disputed case, clarify the actual family basis instead of buying a random policy advertised as “just like NFZ”.
Students, people without work and voluntary insurance
For foreign students, citizenship, residence documents, employment, eligible family inclusion and the university's obligations matter. The official student guidance distinguishes non-EU students from other cases. A student card alone is not universal insurance.
Without a compulsory or other suitable entitlement, investigate a voluntary agreement with your regional NFZ branch. Your category determines the application, identity/residence evidence and previous-insurance records; after contracting, complete the prescribed ZUS steps and contributions. An NFZ agreement is not the same as transferring money to an unknown insurance intermediary.
The ordinary voluntary-insurance NFZ contribution changes quarterly. Eligible non-EU foreign students use a special base giving a contribution of 55,80 PLN; this is not the price for every foreign national. A gap can trigger an additional charge; exemption or instalments depend on the prescribed grounds rather than being automatic.
After employment ends, check insurance termination and any continuing entitlement under the applicable basis. Do not apply one period to employees, students and family members alike. A temporarily red status and no lawful entitlement require different responses; retain records of the previous and new bases.
UKR after March 2026: a separate check is essential
The NFZ explanation from March 2026 confirms the end of the previous general special-act healthcare rights from 05.03.2026. Obtaining PESEL UKR and acquiring your own compulsory health insurance are not the same.
Special access remains for temporary-protection beneficiaries with UKR who survived torture or rape, are under 18, are pregnant, giving birth or in the postnatal period, or hold confirmation of residence in collective accommodation. These bases exclude certain services. For example, do not promise the full spa-care route, infertility treatment, cataract surgery and joint replacement, or particular medicine and screening programmes without checking the special rule.
War-injured people evacuated for treatment have a separate arrangement, including different treatment of joint replacement; it does not apply to everyone crossing the border. Eligible temporary-protection beneficiaries with UKR have access to life-saving services for sudden threats to life or health. That is not universal planned healthcare without another basis.
A person with UKR and their own compulsory insurance, for example through work, or an eligible voluntary agreement receives care under that insurance basis. A special transitional rule for an existing hospital admission ends with treatment and no later than 04.03.2027, subject to separate exclusions; it is not an extension of everyone's insurance until that date.
Check eWUŚ, IKP and documents before the appointment
eWUŚ confirms entitlement to healthcare on a particular day. Its information is updated using ZUS and KRUS records. A green result assists confirmation; red does not always mean no legal entitlement: registration may have failed, details changed or records not yet reconciled.
Check status in Internetowe Konto Pacjenta or the mojeIKP app's insurance section. The official checking instructions explain digital access. An account or app does not create insurance; it displays records. Do not give a receptionist your password or use someone else's PESEL to obtain a green result.
For red status, bring identification and evidence of your basis: payer registration, family inclusion, voluntary agreement and payment, or a special-entitlement document. The patient instructions provide for documentary confirmation or an admissible declaration. Sign a declaration only when you genuinely qualify and the procedure permits it; do not automatically use an insured person's general declaration for every special UKR entitlement.
Without PESEL, ask for the procedure using your own identity document and prescribed evidence. A different technical route may be needed, but lacking the number should not automatically be equated with lacking entitlement. No physical residence card is not a conclusive answer without checking another lawful basis either.

Choosing a POZ family doctor: paper or IKP
POZ means podstawowa opieka zdrowotna, primary healthcare. Choose an NFZ-contracted provider and a doctor accepting new patients. Doctor, nurse and midwife each have a separate deklaracja wyboru; they need not work in the same building. Ask about languages, booking hours and child services.
Submit the declaration on paper at the provider or through the available IKP procedure. Check acceptance afterwards: saving a draft or sending a form is not yet assignment to the chosen doctor. If refused, establish the reason and another acceptable choice. Do not send conflicting declarations merely because a reply is not immediate.
Under the POZ choice rules, two changes per calendar year are free; further changes normally cost 80 PLN. Exceptions include causes outside the patient's control or a change of residence. Do not mistake every first choice for a new paid appointment: changing a declaration and charging for treatment are different things.
Our short doctor-selection and first-visit guide covers registration itself. If away from your usual doctor, establish locally available care instead of automatically changing declarations for a single consultation.
Booking, teleporada and preparing for the conversation
Say whether you need a first appointment, a new problem, follow-up or continuing treatment. Reception organises access but should not replace the doctor's clinical judgment. Describe urgency-related circumstances truthfully; do not invent symptoms to move ahead in the queue.
A teleporada can suit some questions but is not a compulsory replacement for every in-person service. Tell the doctor if a problem cannot be described remotely or examination is needed; rules on consultation format and patient-specific exceptions apply. A call does not guarantee a prescription, antibiotic, L4 or referral from your requested list.
Prepare identification, main concerns and their duration, discharge letters, results, a list of medicine active ingredients and known allergies. Foreign patients benefit from short Polish phrases and an understandable translation of significant medical records. Do not present an unsigned machine translation as another specialist's official opinion.
Cancel if you cannot attend. For a telephone consultation, check the contact number and call arrangements; a missed call differs from refusal of care. Afterwards, make sure you understand the next steps, rather than merely receiving a text-message code.
E-recepta, e-skierowanie and specialist referrals
E-recepta is an electronic prescription; e-skierowanie an electronic referral. They may appear in IKP and mojeIKP and carry usable identifiers, rather than universal rights to free services. A prescription does not always mean medicine reimbursement: indication and payment conditions matter.
With PESEL, a common dispensing route is prescription code plus the patient's number, or an available barcode/QR. Without PESEL, request a barcode information printout issued against your true identity document; do not substitute another person's number. The electronic-prescription checklist explains pharmacy steps.
Most NFZ outpatient specialists need a referral from a health-insurance-system doctor. A private-office referral does not always open publicly funded consultation on the same terms; hospital referrals have their own rules. Ask which system the document belongs to and which service it enables.
For continuing specialist treatment, a new referral is normally unnecessary at every follow-up. Do not automatically return to POZ for tests the treating specialist should organise. Clinical indications and the covered scope are determined by the doctor; insurance does not entitle you to every test you choose.
| Service | General rule | What to clarify |
|---|---|---|
| Cardiologist, neurologist, endocrinologist | An NFZ outpatient referral is usually required | Appropriate referral and urgency category |
| Gynaecologist-obstetrician, oncologist, psychiatrist, venereologist, dentist | General exceptions without referral | NFZ contract and covered scope |
| Psychologist, optometrist, sports-medicine doctor | No referral under changes from 17.09.2025 | Actual availability under the NFZ contract |
| Ophthalmologist, dermatologist | A referral is normally required | Personal special entitlements and separate routes |
| Sudden threat to life/health | Do not wait for a planned referral | Emergency assistance rather than an ordinary queue |
Exceptions were checked against the current Patient Rights Ombudsman announcement and e-skierowanie rules. An old list of only five specialists is incomplete. Some patient groups have additional rights, while addiction and mental-health care can have separate access routes. A referral exception does not guarantee a free slot tomorrow.
Finding an earlier appointment
Use the official NFZ treatment-date information service: choose the service, region and correct category, stabilny or pilny, as recorded by the doctor. The nearest hospital need not have the nearest date. You can look elsewhere, accounting for transport and return visits.
A published date is a provider's indication, not your confirmed booking. Call to establish referral suitability, documents, actual availability and waiting-list confirmation. Where IKP or central electronic registration applies to the service, use its available official route; do not assume it is already compulsory for every specialty.
Do not keep incompatible bookings using one referral at multiple providers. When changing providers, cancel the previous booking and arrange release of the referral. For paper documents, check your original-submission deadline. If health changes while waiting, seek clinical reassessment rather than changing the urgency classification yourself.
POZ, out-of-hours care, SOR and ambulances have different roles
| Route | Purpose | Organisation |
|---|---|---|
| POZ | Ordinary health concerns and continuing care | Primary-care booking appropriate to the circumstances |
| Nocna i świąteczna opieka zdrowotna | Deterioration that should not wait for POZ but is not an immediate threat to life | 18:00–08:00 on weekdays, around the clock on statutory non-working days |
| SOR / izba przyjęć | Sudden threat to health or life | Clinical triage by severity, not arrival order |
| Ambulance: 112 or 999 | A situation requiring emergency assistance | Give location and observed signs and answer the dispatcher |
Out-of-hours care is not a shortcut to a routine referral or ordinary check-up. The patient portal confirms its operating arrangements. In SOR, waiting depends on clinical urgency; arriving earlier does not guarantee being seen before a more seriously ill patient.
For an immediate threat, do not postpone calling while looking for a POZ declaration or insurance printout. The dispatcher needs the exact location and truthful description. This article does not diagnose or replace professional urgency assessment. See the additional evening and night-care checklist.

If reception refuses care or asks for payment
Ask for the specific reason: no entitlement confirmation, no referral, no NFZ contract for this service, or no appointments are different problems. Ask whether a document can confirm your right and who deals with disputed cases. Do not sign a knowingly false declaration or pay an unknown account on a text message alone.
If the employer caused an error, request the specific registration and personal-data confirmation from the payer, then contact ZUS and NFZ if needed. Keep evidence that correction was submitted. Buying a private consultation does not retrospectively fix registration or guarantee automatic reimbursement.
For organisational failures, contact the provider's management first. Contract and NFZ-access questions go to the regional fund branch; patient-rights issues to Rzecznik Praw Pacjenta. Official telephone information: 800 190 590; prepare provider name, date, facts and your request. A complaint does not replace necessary urgent care.
Keep a refusal or your own account of events without publicly exposing other people's medical information. If you paid, retain the contract and invoice: reimbursement depends on the legal grounds, not just a later green eWUŚ result. The official Ombudsman answers explain complaint routes.
Six situations for foreign patients
These are hypothetical routes, not real patient histories or treatment prescriptions.
An employee sees red eWUŚ status
Check identification and employer registration and bring available evidence. With genuine insurance entitlement, clarify documentation or an admissible declaration rather than buying a new policy just because of the screen colour. Do not delay life-saving assistance to correct a record.
A student works under zlecenie
Do not assume the contract creates insurance, especially under 26. Check family inclusion and foreign-student rules and contact the university or NFZ if necessary. A commercial clinic package is not compulsory public insurance.
A parent with UKR brings a child
Check the parent's own insurance and the child's special basis separately. Apply the new rules from March 2026 and establish the category and service scope. Do not automatically extend a minor's special rights to every adult in the family.
A specialist is needed but the queue is long
Check referral and category, consult the official directory and contact another provider. Transfer the booking with cancellation of the old one rather than holding duplicates. If health deteriorates, seek clinical reassessment instead of declaring urgency yourself.
Illness starts in the evening
Distinguish immediate danger from a problem that cannot wait for POZ but does not require life-saving intervention. Use the appropriate out-of-hours or emergency route. Do not go to SOR merely for a prescription or to bypass a planned specialist queue.
No PESEL or physical residence card
Prepare valid identification and evidence of an eligible right and ask for the prescribed procedure. Do not use another person's number or equate a missing card with termination of every insurance entitlement. Separate technical identification from legal eligibility.
Specialist: Mam skierowanie do [poradnia], z kategorią [wpis lekarza]. Jaki jest najbliższy termin w ramach NFZ i jakie dokumenty trzeba przedstawić?
Without PESEL: Nie mam nadanego numeru PESEL. Posiadam [dokument tożsamości] i [potwierdzenie uprawnienia]. Proszę o wskazanie właściwego sposobu rejestracji.
Common mistakes
- Treating PESEL, a residence card or an app as insurance.
- Booking a paid appointment with the same doctor and expecting automatic NFZ payment.
- Ignoring student zlecenie exceptions and renewed family inclusion.
- Relying on an old promise of full cover for any UKR holder.
- Treating red eWUŚ as final or signing a false declaration.
- Obtaining a new referral for every follow-up unnecessarily.
- Keeping duplicate waiting-list places and not cancelling visits.
- Confusing health insurance with L4 sickness payments.
Payment during incapacity has separate conditions. See our L4 guide. NFZ administration also does not replace safe medicine choices: the separate cold and flu medicines guide is not a prescription for every patient.
Prepare a first planned-care route without unnecessary bills
Prepare a small relevant set, not every document about your life in Poland. Bring identification, proof of the applicable right, accepted declaration or referral where needed, and medical records relevant to the concern. For a child, include parent or legal-guardian details. For a foreign document, establish which information the provider must understand and in what format.
Arrange discharge letters chronologically and highlight diagnoses, treatment, significant results and current medicines. Do not discard originals after translating them yourself. If you do not know a Polish substance name, show the package or exact international name rather than substituting a similar brand. Organised information helps the doctor but does not let the patient prescribe tests.
Before accepting a paid service, ask a specific question: is it outside this clinic's contract, outside the covered list, missing a referral, or simply unavailable soon? The next step depends on that answer. You can seek another contracted provider or clarify a referral; sometimes the selected service genuinely is not covered. “The fund will repay everything later” without legal grounds is not a reliable payment plan.
For a test ordered within funded treatment, establish where the referring provider organises it and which attachments are needed. Do not automatically accept “pay anywhere first and we will sort it out later”. Equally, do not demand that any doctor fund a test ordered outside their pathway. Responsibility for diagnostics depends on the particular service and referral.
After the consultation, clarify who issues the referral, books the examination, organises follow-up and provides the result. If results only appear in the provider's system, ask how to access them without giving your password to another person. Old foreign results do not all automatically appear in IKP. A saved discharge summary and clear provider contact reduce repeat paid tests caused by missing records.
Discuss language difficulties when booking. Ask about communication in an available language and a companion's participation under the provider's rules. A free interpreter cannot be promised to everyone, but you should not agree to sign incomprehensible text under pressure either. Request explanation of consent and next steps. A companion assists communication; they do not become a doctor or acquire authority over medical information without a basis.
Keep incapacity documents for work, if issued, separately from your medical records. The employer and healthcare provider have different roles; do not post a full diagnosis to a workplace group chat. Entitlement to consultation, issuing L4 and entitlement to cash sickness benefit are separate questions even when they arise from one illness.
FAQ
Can every foreign national receive free treatment?
No. You need eligible entitlement, a covered service and a provider with the relevant NFZ contract. Citizenship, PESEL and a private subscription alone are insufficient.
Is a residence card required at every visit?
It may be important evidence, but entitlement and confirmation depend on the basis. Do not automatically assume insurance ends without the physical card.
What should I do with red eWUŚ?
Check details and the basis, obtain payer confirmation and present acceptable evidence. A declaration is possible only with genuine entitlement and an applicable procedure.
Can I attend without PESEL?
The prescribed identity document and entitlement evidence can be used. Do not use another person’s number; clarify the technical route with the provider.
Is IKP required for a referral?
No, a digital account is not a condition of every referral. Request the prescribed information or printout and clarify booking.
How do I choose POZ?
Submit a declaration to your chosen NFZ doctor on paper or through IKP and check acceptance. Nurse and midwife choices are separate.
Can I change doctors free of charge?
Two changes per calendar year are free; later ones normally cost 80 PLN, with prescribed exceptions outside your control. This is not a charge for each visit.
Does an ophthalmologist need no referral?
A referral is generally required. Do not confuse this with an optometrist, and check special personal rights. The current exceptions are wider than the old five-specialty list.
Does UKR cover everything until March 2027?
No. New grounds and limits apply from March 2026. A transition date for a particular continuing hospital admission is not universal insurance.
Is a student on zlecenie insured?
Not automatically. Age, student status and other bases matter. Foreign students must check family inclusion, their own work, university or an eligible NFZ agreement.
Can SOR replace a specialist waiting list?
SOR is for emergencies, not bypassing planned queues. Clinicians assess sudden threats; call emergency services when necessary.
Where do I complain after refusal?
Clarify the reason with the provider, then contact NFZ or the Patient Rights Ombudsman according to competence. Record facts without delaying necessary urgent care.
Before the next appointment
Record your entitlement basis and evidence, choose the specific NFZ service and prepare identification and accepted declaration/referral. Check time, address, payment arrangements and attachments. Keep discharge letters and medicine details and cancel unwanted bookings. If refused, establish the actual reason and competent correction route.
NFZ, pacjent.gov.pl and Ombudsman sources accompany the relevant rules. This article does not determine an individual's entitlement; 2027 is not an automatic extension of all current concessions. Next scheduled review — 22.11.2026.
