PESEL and birth registration are not equal to insurance statement
Registration of a birth at USC does not automatically include the child in the parent's insurance. Check on what basis the child is entitled to treatment: as a family member, on its own or under special rules. This is especially important for foreign families: all benefits for Polish citizens cannot be transferred to any situation.
If the parent has the appropriate insurance base, the child is usually declared as a family member through the contributor. Do not pay for "separate child insurance" until you have checked the possibility of such inclusion. If neither parent is insured, contact the NFZ for an assessment of the specific grounds; PESEL this gap does not correct.
Where to transfer data
The employee or contractor under the insured contract informs the employer or client. The entrepreneur submits ZUS ZCNA to ZUS. A registered unemployed person applies to the labour office. For a voluntary insured person, the appropriate insurance must first be made with the NFZ, then the application of a family member.
Ask the HR department for your form and confirm registration. For direct submission, use the official ZUS ZCNA and the available ZUS channel. Keep a copy of the submitted data and the confirmation so that after contacting the clinic, the discrepancy can be quickly clarified.
Prepare information about the child
- Name and surname according to the document.
- PESEL or, in its absence, the details of the required identity document.
- Date of birth and relationship to the insured.
- Address and date of the cause of inclusion.
- Information on the change or termination of another basis of insurance, if applicable.

Don't miss change after new job
The patient portal indicates a seven-day period for reporting/registration in the situations described, in particular after the PESEL has been assigned to a newborn or a new insurance basis has arisen. For the employee, the payer must first be informed; he has a separate obligation to submit the form to ZUS.
When a parent is removed from registration, the child's related registration is terminated. After changing employer, register the child again, even if the PESEL number is the same. Do not assume that the new company will receive family data from the old one automatically.
If the right is not visible in the polyclinic
Ask the payer the date and ID of the submitted application, check PESEL and name. If it has been sent correctly, contact NFZ for confirmation. The problem of electronic representation does not necessarily mean that there is no legal right, but a declaration of that right cannot be signed at random.
Keep the information about the parents and the child's grounds together with the medical file. If your child starts his or her own business, or changes age or education, review the family membership terms. This instruction explains the route of submission; a disputed foreign status requires verification in the NFZ, not the creation of a fictitious registration.
