Do you need a separate application?
First check whether you have been assigned a number before: look in municipal documents and public service records. PESEL is assigned automatically when registering residence for more than 30 days. If address registration is not possible but you need the number on a specific legal basis, submit a separate application.
What is a legal basis?
The form must identify the provision requiring you to have PESEL. Simply writing “for the bank” without explaining the basis is not a substitute. If ZUS or the tax office requires the number, ask it to identify the basis. Do not copy a random legal provision from someone else’s form: it must apply to your situation.
Where to get the form
The official gov.pl page “Uzyskaj numer PESEL” provides the wniosek o nadanie numeru PESEL application form. Download it from the documents section or collect it at the municipality. Check the spelling of your name, your date and place of birth, your parents’ details and your identity document. Sign a completed form, not a blank one.

In-person submission and online services
For a separate application, most third-country nationals must attend any municipal authority in person; the rules differ for some categories. Do not confuse this procedure with electronic address registration, which is available in certain cases. When booking, choose the PESEL service rather than an appointment at the voivodeship office.

The result and correcting errors
The municipality accepts the application immediately and, if there is a basis, issues notification that a number has been assigned. Check the details before leaving. If anything differs, identify the incorrect field, show your document and ask how to correct the record. Do not submit a new application as if you were a different person.
When a different procedure is needed
PESEL UKR is connected to temporary protection and requires checking the relevant status; this guide does not replace UKR registration. An ordinary number does not automatically give you residence rights, permission to work or insurance coverage.
